Responsible Ministry Forms

The 2025–2026 Responsible Ministry year has ended, and new Appendix 8A and Appendix 10 forms are required for all youth volunteers, and youth participating in parish activities this fall. Please see below for the required forms.

• Appendix 8A - Children/Youth Registration, Image Release, and Parent/Guardian Consent
