Note: All youth participant registration forms for VBS must also include a completed copy of Appendix 8A and 10 (see below).

Stewardship Application Form

The 2026-2027 Responsible Ministry year has begun, and new Appendix 8A and Appendix 10 forms are required for all youth volunteers, and youth participating in parish activities this fall. Please see below for the required forms.
